to the Underwriters Group Bond Link.
You’ll find that ordering your bond is quick and
easy.
For
state licensing bonds, print
the application.
• Please complete the information blanks
found on page 1 and Section 2 on page 2.
• Date and sign the application on the last
page. The entity being licensed (applicant) should
sign in the appropriate space as an individual,
partnership or corporation. Each owner should
sign the last section. A witness is required for
all signatures.
• Please mail the application to the Bond
Department, Underwriters Safety and Claims, Inc.,
P.O. Box 23790, Louisville, KY 40223, for issuance
of the bond(s).
• The invoice for each bond will be mailed
to you separately by the Hartford Insurance Company.
Checks should be made payable to Hartford Insurance
and be mailed to them at the address shown on
the invoice. The premium will calculated based
on $10 rate per $1,000 of the bond penalty or
a minimum premium of $100.
